Front-Line Management of Mantle Cell Lymphoma in Canada: A Practical, Risk-Adapted Approach

Résumé

Mantle cell lymphoma (MCL) is a mature B-cell non-Hodgkin lymphoma (NHL) that accounts for approximately 5–7% of all lymphomas.1 Its clinical presentation is heterogeneous, ranging from an indolent leukemic non-nodal subtype that may be managed with observation to highly aggressive disease characterized by TP53 alterations, blastoid or pleomorphic morphology, and high Ki-67 expression. Although relatively uncommon, an estimated 500–600 new cases are diagnosed annually in Canada,2 underscoring the importance of optimizing evidence-based management strategies. In this review, we discuss front-line treatment approaches for MCL within the Canadian practice landscape.

Justin Desroches

Dr. Justin Desroches is a hematologist at the Centre hospitalier de l’Université de Montréal (CHUM). He obtained his medical degree and completed his internal medicine and hematology training at McGill University, followed by fellowships in lymphoma at BC Cancer and cellular therapy at Mayo Clinic Rochester. His clinical and research interests focus on lymphoma and cellular therapy, with a particular interest in exploring the role of the intestinal microbiome in mantle cell lymphoma and optimizing the delivery and outcomes of CAR T-cell therapy.

Diego Villa

Diego Villa is a medical oncologist at the BC Cancer – Vancouver Cancer Centre and Clinical Associate Professor at the University of British Columbia. Dr. Villa is lymphoma co-chair of the Canadian Cancer Trials Group and a member of the US National Cancer Institute lymphoma steering committee. Dr. Villa manages patients with lymphoproliferative diseases and conducts research in B-cell non-Hodgkin lymphomas. His main research interests include mantle cell lymphoma, primary and secondary CNS lymphomas, and the role of the intestinal microbiome in lymphoma. He is the principal investigator of national prospective trials of novel therapies in aggressive lymphomas. Over the years he has collaborated with several international groups on NHL studies, with over 150 publications in the peer-reviewed literature.
